...with external MOLLE webbing so I can hang pistol mag pouches off the front of the M-4 mag pouches? Got a new vest and I am having trouble getting to the pistol mags on my belt and my sub load is already filled, so I don't have much real estate on my leg.
Also - which of the open top M-4 mag pouches are any good?

Paraclete makes a pouch alread sewn together like what you want your end result to be. The SF guys love them...

How's your luck been with TT? The already sewed stuff seems a bit more bombproof than the MOLLE option TT has, but I have no firsthand experience with them as a company.
I've had great luck with TT.
They are right outside the gate of Ft Lewis off I5, I've stopped in the shop several times.
but since my current duties don't have me carrying the big green tick over hill and dale anymore - I more frequently put on gear, stand around, then unloading it all back into the trunk. Don't take my word for it: http://socnetcentral.com/vb/showthread.php?t=56123 :D

I've been using Tactical Tailor mag pouches for a couple years with no complaints at all, I just bought a couple more. I wouldn't call it "hard" use, but I use them almost daily. Their customer service has always been real good, too. The only exception is that I emailed them about a custom job a few weeks ago and never got a response.

TT is good but they don't make a triple-triple

Yeah - I was thinking triple rifle, then it leaves MOLLE on the outside for pistol panel.

Have to say the Paraclete Triple Triple is great. Like Snaquebite said it is exactly what you are asking for. The TT is great as well in that you can buy 3 single M4 pouches and then attach additional handgun mag pouches to this, thus making a triple triple or double double etc. The advantage is that if you want to add other gear, remove a mag pouch, with the Paraclete, you can't. I currently run with the Paraclete and it has performed well. Only draw back is that M4 mags have fallen out when the pistol mag pouch is empty. Either one is great. If not mistaken, the TT option will cost more.

The only issue I'll mention with the Paraclete pouch as shown is that there isn't any "real" retention for the rifle mags. I've got one & depending on mission, it might not have my complete confidence.
What I'm a bigger fan of, and have used for 3-4 years is this one from Eagle:
http://triadtactical.com/store/item/191qk/Rifle_Mag_Pouches/Eagle_MLCS_MP2_M4-4_45-FB1-MS-KHAKI.html
They make them to accomodate single stack 1911 mags & also double stack Glock mags (might fit other 9mm/.40 as well).
The advantage (IMO) to this product is twofold:
1. It has rollover webbing to assist in retaining the mags when you need more than just "friction".
2. The mag pouches themselves are kydex lined with a soft "one-wrap" material in them & have a better friction fit than the elastic webbing on other versions.
3. They are "bombproof". Good shit.
Disadvantages:
1. With the added "bulk" of the kydex linings, the pouch is pretty damn thick. In the 1911 version, it will add about 3" of thickness to your gear & won't compress.
2. The kydex inserts might not be perfect for your mags, although I'm sure that the one-wrap gives it some flexibility.

I have one their open top mag pouches for my next mag. We tested it with an old mag filled with lead during a basic SWAT course. It never got loose in the pouch even when inverted. These guys make a M4/pistol combo pouch.

Do you have an early model or the newer with the internal elastic retention band?
